My services are covered by many insurance plans that offer out-of-network benefits.  I can provide a monthly statement upon request for you to submit for reimbursement in accordance with the terms of your insurance plan. Some insurance plans through which my clients have obtained reimbursement include Aetna, Anthem Blue Cross/Blue Shield, Cigna, Empire Blue Cross/Blue Shield, Horizon, Optum, Oxford, Regence BlueShield, United HealthCare, and TRICARE.

If you are planning on applying for insurance reimbursement, I encourage you to contact your insurance carrier to find out about your benefits. While not all plans will reimburse for my services, many will. When you call your insurance carrier, it may be helpful to have my National Provider ID (1861774705) available. Some questions you may wish to ask:

  • Does my plan cover out-of network mental health services?
  • Are there any restrictions on provider type, or on the type of services covered?
  • What is the usual and customary rate ("allowed amount") covered for psychotherapy services?
  • How much is my deductible and co-insurance?
  • Do I need a referral or pre-authorization?

Cancellation Policy:
48-hour notice is required for rescheduling or cancellation.  Missed appointments that are not cancelled at least 48 hours in advance will be charged at the full fee for the scheduled session. Missed session fees are not covered by insurance.
